HEALTH INFORMATION MANAGEMENT (Medical Records)
HMC Health Information Management (HIM) organizes and manages your health information data by ensuring its quality, accuracy, accessibility, and security in paper and electronic systems. HIM uses various classification systems to code and categorize patient information for reimbursement purposes, databases, and registries and maintain patients' medical and treatment histories.
The Electronic Health Record (EHR) is an electronic version of a patient's medical history that the medical provider maintains over time and may include all of the critical administrative and clinical data relevant to a patient's care under a particular provider, including demographics, progress notes, problems, medications, vital signs, past medical history, immunizations, laboratory data, and radiology reports.
The EHR automates access to information and has the potential to streamline a clinician's workflow. The EHR can also support other care-related activities directly or indirectly through various interfaces, including evidence-based decision support, quality management, and outcome reporting.
EHRs are the next step in the continued progress of healthcare that can strengthen the relationship between patients and clinicians. The data, timeliness, and availability will enable providers to make better decisions and provide better care.

HOW TO REQUEST A COPY OF YOUR MEDICAL RECORDS
Requesting a copy of your medical record is easy. You can self-enroll for access to the HMC Patient Portal. From the HMC Patient Portal, you can view, download, and print laboratory and radiology results and various diagnostic results.

Click as indicated to create a new account.

Enter your data to confirm your identity, and you are all set.

HIM is here to serve you from 8:00 am until 4:30 pm Monday through Friday. Contact us by phone at 731-926-8361. When requesting a CD of imaging, please allow one hour for processing. Your medical records are available electronically or in a printed format.
